Iirc, while you hit Zero hp, roll a d20. I imagine it is a DC 10 con save, or a 50/50 roll. Either approach, if you're making it, you get one "save". If you fail, you get a "failure".
Saves and fails are tracked separately, but at the similar time. One your turn within the initiative, make the save once more, mark the end result. Iirc, if you're taking damage, that is an automated failure.
Once you get Three saves, you're strong at Zero hp, and do not make any more rolls. You can only be killed by a coup de gras, and if any individual heals your hp, you'll be able to re-enter the battle (I don't believe they want to watch for the three saves).
If you get Three fails, you're useless, and only a resurrection spell will save you.
